Intensity Modulated Radiation Therapy (IMRT) for Nasopharyngeal Carcinoma—The Koo Foundation Sun Yat-Sen Cancer Center Experience

ABSTRACT

 

Purpose:

 

To evaluate the experience at Koo Foundation Sun Yat-Sen Cancer Center in using intensity modulated radiation therapy (IMRT) for the treatment of nasopharyngeal carcinoma (NPC).

 

Patients and Methods:

 

Between Jan 2003 and June 2005, 210 patients with histologically proven and non-disseminated NPC were treated with IMRT in our institution. All patients had magnetic resonance imaging (MRI) for the evaluation of their head and neck tumors. Among them, 93% had concurrent chemotherapy, and 76% had post-radiation chemotherapy. Of the patients included in the analysis, male and female ratio was 2.3:1. Seventy-seven percent (77%) of these patients were of WHO type III histology. Twenty-seven percent (27%) had T4 disease, and 10% with N3 disease.

 

Results:

 

The gross tumor volume (GTV) in all patients averaged 60.5ml (ranging from1.0 to 339.0 ml). The median dose to the GTV ranged from 58.2 Gy to 84.7 Gy (median 76.6Gy).   On average, V26 of right parotid glands was minimum 6.5 ml, maximum 46.0 ml and median 19.6 ml, V26 of left parotid glands was minimum 4.0 ml, maximum 44.6 ml and median 18.6 ml. V60 of brain stem was minimum 0 ml, maximum 1.55 ml and median 0.05 ml. V50 of spinal cord was minimum 0 ml, maximum 1.53 ml and median 0.07 ml.

 

Conclusions:

 

IMRT could deliver more dose to the GTV while reducing radiation dose to critical organs such as brain stem, spinal cord, parotid glands and inner ears.
