The prescription, recording and reporting of radiation therapy dosage is critical in the accurate delivery of radiation treatment to cancer patients.  Various, sometimes ambiguous methods have been used in the prescription and reporting of radiation dosage.  Such methods include, among others, prescription of dose to isocenter or a similar point within the target in the low dose gradient region, or as a percentage of local or global maximum doses.  Several issues needs to be carefully considered before consistent and unambiguous methods of dose prescription and reporting can be devised and implemented, prominent among them are the clear definition of target tissue to be treated.  With the introduction of advanced radiotherapy delivery techniques such as Intensity Modulated Radiation Therapy (IMRT), the methodology of dose prescription and reporting can become an integral part of dose optimization, placing an even more important role on how the prescription dose is specified.  This session reviews the recommendations of the ICRU reports number 50 and 62, augmented by practical examples of how the targets are defined for specific treatment sites.  A literature review is provided that contains data on margins of target volumes for typical disease sites.

       放射治疗剂量的处方、记录和报告对于准确实施放射治疗是很重要的。各种各样的,有时甚至是模棱两可的方法被用于放射剂量的处方和报告,包括剂量处方规定在等中心或靶区低剂量梯度区中类似的点,或以局部或全局最大剂量的百分之多少来规定。在一致的、明确的剂量处方和报告方法制定和执行之前,必须认真考虑几个问题,其中最突出的是明确定义要治疗的靶区。随着先进的放疗实施技术(如IMRT)的应用,剂量处方和报告的方法学成为剂量优化的组成部分,对如何规定处方剂量起到很重要的作用。本讲回顾ICRU 50 62号报告的建议,举例说明如何定义特定治疗部位的靶区。对于典型的病变部位,通过文献回顾,给出靶区边界的数据。
